Parents/Guardians at NAMS:
This Google Form should be completed in order to sign your 7th or 8th grade child into Band, Chorus, or Orchestra at Nazareth Area Middle School (NAMS). If you have not done so already, please complete all information required to ensure your child is scheduled for the correct group(s) for the 2025-2026 school year.
Note: Students can be involved in more than one group. Band and Orchestra meet on the same day, but chorus will meet on the opposite day for Class.
