    • Attendance Excuse Form - to report a student absence from school  
    • The Appointment Dismissal Form is no longer in use.  If your child is leaving early for an appointment, please call the attendance office at 610-849-0874 or email hsattendance@nazarethasd.org.
    • Educational Family Trip Excusal Form - to be filled out when a student will be absent due to a trip  
    • College Visit Form - to be filled out by the student prior to the college visit. Please bring verification of the visit to the Attendance Office when returning to school.
