    Absence - Please report a student’s absence by (1) calling the Attendance Hotline at 610-849-0874 or (2) utilizing the Attendance Excuse Form or (3) emailing hsattendance@nazarethasd.org. 

    Attendance Excuse Form - This form may be used to report a student’s full day absence from school.

    Appointment Dismissal - If you need your student dismissed during school hours for an appointment, you may inform us by (1) calling the Attendance Hotline at 610-849-0874 or (2) sending a note in to the Attendance Office with your student or (3) emailing hsattendance@nazarethasd.org. Please give as much notice as possible to allow for processing.

    College Visitation Form - To report an absence due to a college visit, this form must be filled out by the student prior to the college visit. Please bring verification of the visit to the Counseling or Attendance Office when returning to school.

    Family Educational Trip Form - This form may be used when a student will be absent due to a trip.
