Documentation Required for Substitute Employees:
Transcripts - if applying for a Substitute Teacher Emergency Permit, must be an Official Bachelor’s Degree Transcript in a sealed envelope from the college; otherwise can be a copy.
Copy of PA License or PA Teaching Certification (for Nurse or Teacher position only)
Three (3) Required Clearances - All Clearances must be current within one year.
- PA Criminal History Record/Act 34 - Print out and submit a copy.
- PA Child Abuse History Clearance/Act 151. Click on Create an Individual Account. Click on Child Welfare Portal. Print out and submit a copy.
- FBI Background Check/Act 114. Click on Digital Fingerprinting. Enter 1KG6XN as the service code. Click on Schedule/Manage Appointment Go through the screens and enter information as required. Print out and submit a copy or submit a copy of the receipt you receive after being printed.
PA School Personnel Health Record Form - Physical and TB results must be within one year.
Employment Eligibility Form I-9 - Please note the document(s) you must provide to establish identity and work authorization.
