Nazareth Area High School Transcript Release Form
This form is for graduates of NAHS. If you are a current student in need of a transcript, please contact your school’s guidance office. Please note as per district policy, records are retained for a period of thirty years and destroyed thereafter.
Official Transcripts - will be mailed or emailed directly to a school or employer.
Unofficial Transcripts - will be sent by US mail.
Instructions:
- Please fill out & sign the Transcript Release Form - Handwritten signature required, typed signatures will not be accepted.
